The Woodworth Community Health Center is a family favorite for residents in Rapides Parish. Conveniently located off I-49 on U.S. Highway 165 North, the clinic has been serving patients living in the Woodworth-Alexandria area since 2007. Originally known as Woodworth Family Medicine, the practice became part of the Access Health Louisiana (AHL) Network in April of 2016.
While ownership of the clinic and the clinic’s name changed, the caring staff stayed in place to maintain continuity within the community. Woodworth Community Health Center patients love Dr. David Guillot who has been with the clinic since its origination. Patients living as far away north as Shreveport and Bossier City drive more than 100 miles to see Dr. Guillot and his caring staff.

With convenient hours of operation and same day appointments available, scheduling a visit with our providers is easy. Services at the Woodworth Community Health Center include: Adult and Pediatric Primary Care, as well as Behavioral Health. Our team of trained therapists, psychiatrists, and psychiatric nurse practitioners offer individual and group therapy with in-person and virtual appointments. Individuals dealing with opioid use disorder can participate in our medication assisted treatment program under the direction of Leigh Ann de Mondredon, C-ANP. Patients can also be seen for maternal childcare coordination, contraceptive care, LGBTQ+ care, HIV/STI testing, chronic care management, and health coaching.

The Woodworth Community Health Center is recognized nationally for its Diabetes Prevention Program. The “Healthy Lifestyle Program” is a one-year program that helps patients lose weight through improved diet and exercise, and reduce the risk of developing type 2 diabetes by 58%, as a result. The curriculum is proven to be effective and keeps you on track to being successful. Our Woodworth Health Coaches, Johannah and Kami Holden have seen great outcomes with previous groups who surpassed their weight loss goals by the end of the program.

Participants in the 2024 cohort reached their weight loss and A1C goals only 6 months into the program. Reducing your risk of getting diabetes is important because 86 million Americans have prediabetes and 90% of them do not know they have it. Prediabetes means having higher than normal blood glucose levels, but not high enough to be diagnosed as diabetes.

Another benefit of being an Access Health Louisiana patient is that you can save money on your medications through our discount pharmacy program. This allows AHL patients to purchase their prescription medications from local pharmacies at a reduced rate.
While most commercial insurances, Medicaid and Medicare are accepted, patients are never turned away for care due to their inability to pay. A discount sliding fee scale is offered based on a patient’s household size and income. Uninsured patients can receive Medicaid application assistance during normal business hours at the clinic.
