Conveniently located on the St. Bernard Parish Hospital campus along West Judge Perez Boulevard, sits the St. Bernard Community Health Center.  This clinic in Chalmette offers patients a wide variety of services all housed in one location.  Services at this site include:  Adult and Pediatric Primary Care, Behavioral Health, WIC and even diabetes education classes.  Unique to the St. Bernard location is the Industrial Medicine department.  This department helps patients needing Department of Transportation physicals, Non-DOT physicals, Return to work confirmations, Workman’s comp, drug testing, Pulmonary function tests, Audiometric testing, as well as Respirator FIT tests.

Our clinic was recently asked by St. Bernard Parish leaders to take over its WIC (Women, Infants and Children) department.  WIC staff helps moms and mothers of young children gain access to nutritious food through the government’s supplemental food program.  Parents and children must meet specific guidelines outlined by the government.  Applying for benefits is easy and can be done on site at the clinic.  Click here to find out what you need to apply for benefits.

This St. Bernard Community Health Center is open six days a week which makes scheduling appointments, even same-day visits, easy for patients.  For patients needing prescription medications, you can access discounted prescription drugs through the federal 340B program.  While most commercial insurances, Medicaid and Medicare are accepted, patients are never turned away for care due to their inability to pay.  A sliding fee scale is offered based on a patient’s household size and income.

High standards of care, affordability and convenience are just some of the reasons why the St. Bernard Community Health Center was recognized by the National Committee for Quality Assurance as a Patient-Centered Medical Home.  Our physicians and staff, many of whom are also bilingual, continually exceed national standards by offering higher quality of care at lower costs.  This means a better overall healthcare experience for you our patient.

The St. Bernard Community Health Center is a member of the Access Health Louisiana (AHL) Network, the largest network of Federally Qualified Health Centers in Louisiana.
