Conveniently located off of I-55 and Club Deluxe Road in Hammond, the Tangipahoa Community Health Center serves patients living throughout the Ponchatoula-Hammond-Amite-Livingston-Kentwood-Covington region. With convenient hours of operation and same-day appointments available, our providers offer patients a variety of services Monday through Friday from 8am to 4:30pm.
The Tangipahoa Community Health Center team welcomes new and existing patients of all ages.
Our services include: Adult and Pediatric primary care, sports physicals, and immunizations, as well as Flu and COVID shots. Patients can also be seen for maternal childcare coordination, contraceptive care, LGBTQ+ care, HIV/STI testing, chronic care management, and health coaching. For patients seeking behavioral health services, our team of trained therapists, psychiatrists, and psychiatric nurse practitioners offer individual and group therapy with in-person and virtual appointments. Individuals dealing with opioid use disorder can participate in our medication assisted treatment program under the direction of Leigh Ann de Mondredon, C-ANP.

As a patient of the Tangipahoa Community Health Center, you will have access to high tech capabilities including iPad patient registration. There is also an online portal where patients can communicate with their physicians, access lab results and pay their bill with the touch of a button. We also have bilingual staff available to help make patient-provider communication easier for Spanish-speaking families.
The Tangipahoa Community Health Center’s behavioral health department is also one of the only facilities in the region that specializes in using Transcranial Magnetic Stimulation (TMS) to treat depression and anxiety. TMS is focused magnetic energy directed to the left side of the brain’s front cortex. Our skilled providers also specialize in treating:

For added convenience, we also offer telehealth visits for our patients. Patients can speak to their provider from the convenience of their desktop or mobile device during their scheduled appointment time. Telehealth is the perfect complement to on-going patient care. In fact, patients can speak virtually with their psychiatrist about a prescription or refill, therapeutic options or assist in scheduling child or senior care appointments. Certain behavioral health services must be provided in one of the Access Health Louisiana offices. Any initial assessments or psychological testing must be done on-site. In the early stages of evaluating and diagnosing your condition, your provider needs to see you in person. This allows your provider to observe your overall health and behaviors. It’s also a good way for you to begin to develop your relationship.
While most commercial insurances, Medicaid and Medicare are accepted, patients are never turned away for care due to their inability to pay. A discount sliding fee scale is offered based on a patient’s household size and income. Another benefit of being an AHL patient is that you can apply for Medicaid benefits right on site. Patients can also purchase prescription medications at a reduced rate through our discount pharmacy program. Most prescriptions can be filled at convenient local pharmacies right where you live. Patients may also use the Access Health Louisiana Pharmacy which offers free home delivery of medications.
