This healthcare hub located in the historic Broadmoor neighborhood houses a diverse menu of services for patients to choose from including: Adult and Pediatric primary care. Patients can receive immunizations including Flu and COVID shots, sports physicals, as well as annual well visits. Patients can also be seen for maternal childcare coordination, contraceptive care, LGBTQ+ care, HIV/STI testing, chronic care management, and health coaching. For patients seeking behavioral health services, our team of trained therapists, psychiatrists, and psychiatric nurse practitioners offer individual and group therapy with in-person and virtual appointments. Individuals dealing with opioid use disorder can participate in our medication assisted treatment program under the direction of Leigh Ann de Mondredon, C-ANP. For patients trying to lose weight, we offer convenient appointments with our health coach. Health Coaches help patients set attainable goals and teach them how to achieve their desired outcomes.


In addition to taking most commercial insurances, Medicaid and Medicare, the South Broad Community Health clinic never turns a patient away due to their inability to pay. We offer a discount sliding fee scale based on the patient’s household size and income. The clinic also serves as a Medicaid Application site. Medicaid applications are taken daily during clinic hours.

This means that you can get your prescriptions filled at most local pharmacies and save money. We accept most commercial insurances, Medicaid and Medicare. If you are uninsured, you won’t be turned away for service. You can still see a provider by utilizing our sliding fee scale. This means that the cost of your visit will be based on your household size and income. For some patients, this means seeing a doctor for as little as fifteen dollars.
