Patients can also be seen for maternal childcare coordination, contraceptive care, LGBTQ+ care, HIV/STI testing, chronic care management, and health coaching. For patients seeking behavioral health services, our team of trained therapists, psychiatrists, and psychiatric nurse practitioners offer individual and group therapy with in-person and virtual appointments. Individuals dealing with opioid use disorder can participate in our medication assisted treatment program under the direction of Leigh Ann de Mondredon, C-ANP. For patients trying to lose weight, we offer convenient appointments with our health coach. Health Coaches help patients set attainable goals and teach them how to achieve their desired outcomes.
In addition to taking most commercial insurances, Medicaid and Medicare, the South Broad Community Health clinic never turns a patient away due to their inability to pay. We offer a discount sliding fee scale based on the patient’s household size and income. The clinic also serves as a Medicaid Application site. Medicaid applications are taken daily during clinic hours.
