The Oakdale School-Based Health Center (SBHC) is located in Allen Parish on the campus of Oakdale Middle and Oakdale High School. The 2,100 square foot clinic is easily accessible for elementary, middle and high school students because it is centrally located in the middle school building and connected by breezeways. Students can visit the health center during the school day outside of their core classes. Parents may request that their child receive immunizations or check-ups by communicating information to the Medical Assistant or Nurse Practitioner on staff. All students must have a current consent and enrollment form on file at the health center in order to be seen by medical staff. Consent and enrollment forms are sent home at the beginning of the year; however, parents may sign their child up for services at any time during the school year. You can download copies of both forms from this web page or obtain a copy through the school office.
Nurse Practitioner Jessica Floyd Thibodeaux offers children and adolescents immunizations, well & sick visits, sports physicals, as well as acute and diagnostic care. Mrs. Thibodeaux enjoys working in school-based healthcare. She is assisted by Medical Assistants Tyra Blow and Sadie Fuselier, as well as Registered Nurse, LaJuanna Nash. Ms. Nash also serves as school nurse. Licensed Clinical Social Worker Jessie Deville offers counseling and support services for students including: stress and anger management, dealing with anxiety, bullying, substance abuse, grief counseling and communication. The School-Based Health Center is managed on site by Sadie Fuselier.
Students needing emergency care after hours, please call 9-1-1. For students needing to continue their care throughout the summer or on school breaks, visit Dr. David Guillot or Nurse Practitioner Randalla Reynolds at the Woodworth Community Health Center (CHC) located at 9372 Highway 165 South in Woodworth (just 20 minutes from Oakdale). The Woodworth CHC is a member of the Access Health Louisiana Network. Students registered with Access Health Louisiana through the Oakdale SBHC do not need to re-register for care when seen at the Woodworth CHC.
