Plaquemines Parish residents rely on Dr. Patrick Johnson and Nurse Practitioner Jillian Ashbolt at the Belle Chasse Community Health Center for their preventive and diagnostic care. Dr. Johnson is board certified in internal medicine and pediatrics. He also serves as Associate Vice President of Medical Affairs for Access Health Louisiana.
Located along Highway 23 in Belle Chasse, the clinic offers same-day appointments and convenient hours of operation. All members of the family can be treated at our community health center. We offer adult and pediatric primary care including immunizations, sports physicals, and well child visits.

Patients can also be seen for maternal childcare coordination, contraceptive care, LGBTQ+ care, HIV/STI testing, chronic care management, and health coaching. For patients seeking behavioral health services, our team of trained therapists, psychiatrists, and psychiatric nurse practitioners offer individual and group therapy with in-person and virtual appointments. Individuals dealing with opioid use disorder can participate in our medication assisted treatment program under the direction of Leigh Ann de Mondredon, C-ANP.

As a patient of the Belle Chasse Community Health Center, you will have access to discounted prescription medications through the federal 340B program. This means that you can get your prescriptions filled at most local pharmacies and save money. Patients may also use the Access Health Louisiana Pharmacy which offers free home delivery of medications.
We accept most commercial insurances, Medicaid and Medicare. If you are uninsured, you won’t be turned away for service. You can still see a provider by utilizing our sliding fee scale. This means that the cost of your visit will be based on your household size and income. For some patients, this means seeing a doctor for as little as fifteen dollars. Patients can also apply for Medicaid applications at the clinic during regular business hours.
