The American Heart Association has recognized Access Health Louisiana in our work around cholesterol management, Type 2 Diabetes prevention, and blood pressure control.

AHL’s work with the 2022 Check. Change. Control. Cholesterol Recognition Program has earned us Gold Level status from the American Heart Association. Within our at-risk patient population, AHL had greater than 70% statin therapy use. Senior Vice President of Population Health for Access Health Louisiana, Chatrian Kanger said that ” promoting medication adherence through the use of our in-house pharmacy,” was integral to AHL’s accomplishments.

 

In the category of improving patient blood pressure control, the American Heart Association awarded us Silver Level status for our accurate measurement among adult patient populations. This target aims to tackle uncontrolled blood pressure by measuring blood pressure accurately; acting rapidly with a clear treatment plan; and partnering with patients to enable ongoing self-management. Kanger said about the program “Our teams here at Access have learned so much through our participation in the American Heart Association’s Target BP initiative over the past several years.  Through our involvement with AHA’s initiatives, we have adopted evidence-based guidelines for management of hypertension, refined internal protocols and referral processes, as well as promoting medication adherence through the use of our in-house pharmacy.”

AHL’s dedication to improving quality of care through awareness and management of type 2 diabetes and cardiovascular risk factors earned us participant recognition status from the AHA as a part of the 2022 Target: Type 2 Diabetes.
